Prior to admission to Oakview, our staff work closely with the clients’ care team to ensure that the process runs smoothly. We always request recent client labs, EKG, PPD, vitals, prior discharge summary, and any medications the client is taking. Our psychiatrist will then review the client’s case to ensure that it is safe for them to be admitted to the facility. Once approved, we will coordinate with you regarding the client’s transport to the facility for intake. A parent or guardian will need to be present with the client for the intake process.
Our team is well-aware of the need to coordinate with clients’ previous and future care professionals to provide proper continuity of care. Our clinicians often reach out to past providers in order to gather pertinent information for the client’s treatment. We also coordinate with future external treatment providers as needed to discuss discharge planning as clients approach the end of their stay at Oakview. Overall, the goal is to ensure a smooth process for client, family, and external providers from the time of referral to handoff at discharge.
